Member Agreement

Your membership automatically renews until terminated. Your membership will continue month-to-month following the end of your membership contract term unless you terminate the membership.

  • Early termination of your membership contract incurs a $50 administrative fee plus 50% of the remaining contract value.

    • For example, if you cancel a your 3-month contract with 2 months remaining and your monthly membership rate is $150, you would be responsible for:

    • Administrative fee: $50 Remaining contract obligation: (2 months × $150) × 50% = $300

    • Total early termination fee: $350

  • To ensure a premium experience, any changes to your membership, such as an upgrade, downgrade, freeze, or cancellation must be provided in writing. Please email hello@laformefitness.com and a member of our Studio Operations team will work with you on the request during our business hours.

  • Unused classes in a monthly membership do not roll over to the next month.

  • Members are allowed to take up to two (2) classes per day.

  • Late cancellation (within 12 hours) or a no-show incurs additional fees. Please see the Studio Policies page https://www.laformefitness.com/studio-policies for the late cancellation and no-show fee information. 

  • The waitlist has automatic enrollment up to 60 minutes before a class. If you are on the waitlist and then late cancel or no-show to that class, a late cancellation or no-show fee will apply. 

  • "Waitlist" counts as a regular reservation. If you no longer want to be on the waitlist, remove yourself to avoid any potential  late cancellation or no-show penalties or fees.

  • Standby spots open up 5 minutes before class/at the start of the class. Standby is not the same as a waitlist spot. Standby is someone who is not on the waitlist but is physically present at the studio and waiting for a potential spot to open in a class. 

  • Late arrival of 5 minutes or more may result in forfeiture of spot and loss of a class credit. Please see the studio policies page - https://www.laformefitness.com/studio-policies - for full details on late arrivals. 

  • Instructors and classes are subject to change; we will provide updates in the app, the website, and/or by text, and/or by email if there are changes that impact your scheduled class 

  • All classes must be paid prior to taking a class. We are a cashless studio and all classes must be prepaid through a package or membership.

  • You have the right to cancel this membership contract without penalty within three (3) business days of signing the member agreement. This cooling-off period is guaranteed by Missouri state law.

  • All sales are final. If there are medical or extenuating circumstances, please email  hello@laformefitness.com and a member of our Studio Operations team will work with you on the request during our business hours.

  • You may not record or take photos during a class without prior written consent from the studio manager.

  • La Forme accepts major credit/debit cards; we are a cashless studio.

  • Policies and rates are subject to change; we will inform you should policies or rates change with a minimum 30 days notice.

  • To cancel your membership, go to ‘Cancel Plan’ on our website or app. Your cancellation will be effective at the end of your current billing cycle. The cancellation process is as easy as signing up. You will receive a confirmation email upon successful cancellation.

  • You understand and agree to the Studio Policies found at https://www.laformefitness.com/studio-policies.

  • You understand and agree to the Liability Waiver that releases and discharges La Forme Fitness, its owners, partners, employees, instructors, and agents from any and all claims or damages arising from participation in the aforementioned exercise classes or the use of the equipment or facilities.
